Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective, equitable value-based primary care to adults on Medicare. You will practice independently within a collaborative care team, focusing on prevention, care coordination, and population health.

As part of our outpatient clinics, you will conduct visits, wellness exams, and coordinate with physicians and specialists to keep patients healthy and out of hospital.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
