Job summary

Oak Street Health is seeking a Nurse Practitioner for our outpatient team to deliver value-based primary care to adults on Medicare. You will work with a collaborative care team, provide longitudinal care, and lead population health efforts within the clinic.

The role emphasizes independent patient visits, care coordination with physicians and specialists, and participation in wellness visits, documentation, and outreach activities. Must have NP credentials and active DEA license.

Qualifications

• MSN, DNP, or NP certificate required.
• National certification in an NP specialty.
• Active NP license in US and DEA license.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers and facilities.
• Population health leadership with the care team.
• Phone triage and outreach support.
• Educating patients on health conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
