Job summary

Qualifications

• Candidate must hold MS in Nursing, DNP, or NP certificate program.
• National certification in Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary NP license; valid DEA license; US work authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, labs, and care managers.
