Job summary

Oak Street Health seeks a Nurse Practitioner to deliver value-based primary care to Medicare patients in an outpatient setting. You will work on a collaborative care team with physicians, nurses, and staff to ensure coordinated, high-quality care.

You will conduct visits, perform wellness assessments, coordinate with specialists, and support population health goals while educating patients about their health plans and treatments.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate required.
• National certification in Family NP / Adult-Gerontology / Adult / Gerontological NP.
• Active state NP license and active DEA license.

Responsibilities

• Provide effective, value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Coordinate care with a multidisciplinary team and external specialists.
• Educate patients on health plans, conditions, and treatment options.
• Participate in home visits and outreach activities as required.
• Contribute to population health initiatives and quality improvement.
