Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective, value-based primary care to adults on Medicare. You will work within a collaborative care team to provide longitudinal care, conduct wellness visits, and coordinate with specialists to improve population health.

You will lead care coordination efforts, educate patients, and participate in outreach and promotional activities while maintaining independence with physician collaboration.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in at least one listed NP specialty
• Active, non-probationary state NP license
• Active DEA license
• US Work Authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.

NP Certificate Program

National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
