Job summary

Oak Street Health seeks a Nurse Practitioner to provide effective, value-based primary care to adults on Medicare in our outpatient clinics. NPs work with a collaborative team including physicians, PAs, medical assistants and care managers to deliver comprehensive care and build lasting patient relationships.

The role emphasizes care coordination, wellness visits, and population health with cross-functional collaboration. Eligible for a competitive benefits package and full-time schedule.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program completion.
• National certification in one of: Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license and active DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties as assigned.
