• National certification in one of: Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary NP license and DEA license required.
• US work authorization must be verified.

Responsibilities

• Provision of exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with physicians, specialists, labs, and care managers.
• Population health leadership in coordination with the care team.
