Job summary

gobravvo is seeking a Primary Care Physician (MD/DO) in Port St. Lucie, FL to join a value-based care model focused on preventive, patient-centered medicine for adults 65+.

You will work with an integrated care team to manage chronic disease, promote wellness, and deliver high-quality outcomes. The role offers a competitive base salary, annual performance bonuses, a sign-on incentive, and comprehensive benefits starting on day one.

Qualifications

• MD or DO with board certification/eligibility in Internal Medicine or Family Medicine

Responsibilities

• Provide comprehensive primary care to a Medicare Advantage population
• Focus on prevention, chronic disease management and wellness initiatives
• See approximately 15 patients per day to allow for quality interactions
• Collaborate with NP/PA, Medical Assistants, Social Workers, and other team members
• Utilize integrated lab, pharmacy, and referral coordination resources
• Participate in quality improvement and value-based care initiatives
