Qualifications

• MD or DO from an accredited medical school.
• Board Certification or Eligibility in Internal or Family Medicine.
• Experience with geriatric care and Medicare Advantage preferred.
• Active, unrestricted medical license.

Responsibilities

• Provide outpatient, preventive and chronic care to adults 65+.
• Manage a manageable patient census with thorough assessments.
• Collaborate with interdisciplinary care team to ensure holistic care.
• Document care in the EMR and participate in quality initiatives.
