Job summary

gobravvo is seeking a dedicated Primary Care Physician (PCP) to join a 100% outpatient, value-based care practice in Port Orange, FL. This full-time role focuses on preventive medicine, chronic disease management, and patient-centered care for adults 65+.

No productivity quotas are enforced, allowing meaningful patient relationships. You will work with a fully integrated care team including NPs, PAs, MAs, social workers, and in-house labs/pharmacy, with a patient panel averaging 15–17 visits

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in Internal Medicine or Family Medicine.
• Valid state medical license (or ability to obtain).
• Experience in value-based care models preferred but not required; openness to learning VBC.

Responsibilities

• Provide comprehensive primary care to adult patients with focus on preventive health and chronic disease management.
• Collaborate with an integrated care team including NPs, PAs, MAs, social workers, and in-house pharmacy/lab staff.
• Manage a patient panel of ~15–17 visits per day with thorough, individualized care.
• Participate in development and implementation of patient-centered care plans.
• Utilize value-based care strategies to improve outcomes and satisfaction.
