gobravvo is seeking a Primary Care Physician (MD/DO) to join our value-based care team. You will deliver comprehensive, patient-centered primary care to an adult Medicare population in a modern center with on-site labs and telehealth support.

Qualifikationen

• MD or DO from an accredited medical school.
• Board certification or eligibility in Internal Medicine or Family Medicine.
• Active, unrestricted medical license in good standing.
• Fellowship in geriatrics preferred but not required.
• Bilingual Spanish highly valued.

Aufgaben

• Provide primary care services, including preventive care and chronic disease management.
• Manage a patient panel with an average daily volume of 13–16 visits.
• Work with multidisciplinary teams to coordinate patient care.
• Utilize EMR platforms (Canopy/Greenway) and on-site labs for care coordination.
• Offer telehealth input from specialists for complex cases.
• Lead patient education and wellness initiatives.
