gobravvo is seeking a compassionate Primary Care Physician (PCP) to join our value-based care team focused on adults 65+ in Palm Beach, FL. This full-time role emphasizes preventive medicine, chronic disease management, and building lasting patient relationships in a collaborative, patient-centered environment.

You will work with NPs, PAs, Medical Assistants, Social Workers, and Pharmacy/Lab staff to coordinate care, seeing 12–15 patients per day, guiding quality-improvement efforts, and

Qualifications

• Board-certified or board-eligible in Internal Medicine or Family Medicine.
• Experience with geriatric populations preferred.
• Familiarity with HEDIS and value-based care models a plus; training available.

Responsibilities

• Provide primary and preventive care to adults 65+.
• Manage chronic conditions and coordinate care with an integrated team.
• See 12–15 patients per day to ensure quality care.
• Participate in quality improvement initiatives and achieve value-based care goals.
• Engage in patient education and health promotion.
• Utilize EHR and coordinate care with referrals.
