Job summary

gobravvo in upstate South Carolina is seeking a dedicated Primary Care Physician (MD/DO) to deliver patient-focused primary care for adults aged 65 and older, emphasizing preventive and chronic disease management within a value-based care framework.

Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine.
• Experience in geriatric medicine is preferred but not required.
• Familiarity with HEDIS measures considered an asset.

Responsibilities

• Deliver patient-focused primary care to adults 65+, prioritizing preventive and chronic disease management.
• Spend 20-40 minutes per patient visit to address complex needs and build strong provider-patient relationships.
• Collaborate with a multidisciplinary team including NPs/PAs, medical assistants, social workers, and in-house lab/pharmacy support.
• Engage in an integrated, team-based care model to coordinate referrals and ensure comprehensive care.
• Utilize EMR efficiently and adhere to documentation best practices.
