Chamber is seeking a Practice Success Manager to partner with network practices, focusing on value-based care, clinical performance, and technology adoption. You'll collaborate with physicians, practice managers, and care teams to improve patient outcomes by interpreting performance data, closing care gaps, and delivering scalable enablement across product and data teams.

Remote role with periodic travel to practice sites as needed.

Formación

• 5–8+ years in customer success, practice management, or provider engagement within value-based care, managed care, or population health.
• Strong understanding of clinical workflows, quality metrics, and payer-provider collaboration.
• Excellent relationship management and communication skills with a consultative approach.

Responsabilidades

• Serve as primary relationship owner for assigned practices, driving value-based goals.
• Train and coach providers on Chamber’s platform and ensure tool adoption for care coordination and reporting.
• Coordinate onboarding and success planning for each practice, delivering progress reports to stakeholders.
• Monitor engagement metrics and identify opportunities to improve outcomes and utilization.
