Job summary

Aylo Health is seeking a Network Performance Manager to guide primary care practices through value‑based transformation, improving HEDIS results, population health, and cost efficiency. You will partner with practices to optimize workflows, support care coordination, and drive sustainable, high‑quality care.

You will coach staff, analyze data, and lead reporting to leadership while traveling 25–50% to practice sites and occasionally visiting Atlanta/Nashville offices for strategic sessions.

Qualifications

• 4–7 years of healthcare transformation and clinical quality improvement experience.
• Experience with primary care practices or practice facilitation preferred.
• Strong coaching and communication skills.

Responsibilities

• Partner with primary care practices to assess baseline performance and co-create transformation strategies.
• Guide practices through HEDIS quality improvements and payer-aligned initiatives.
• Coach clinical and administrative staff on quality improvement methodologies.
• Analyze clinical and operational data to inform decisions and monitor progress.
• Assist practices in leveraging EHRs and population health tools for care coordination.
• Facilitate virtual coaching, learning opportunities, and site visits to share best practices.
• Act as liaison between primary care practices and payers on performance and VBC initiatives.
• Prepare leadership reporting packages related to contract performance.
