Job summary

Vytalize Health is seeking a Practice Transformation Manager to drive on-the-ground implementation of value-based care with assigned provider practices. You will lead practice transformation, optimize workflows, and ensure adherence to quality initiatives that improve patient outcomes and contract performance.

You will collaborate with Medical Directors and regional teams, manage multiple practice relationships, and leverage data to guide interventions while ensuring compliance with regulatory

Qualifications

• 3+ years in practice transformation, care coordination, population health, or related healthcare ops role.
• Experience working with physician practices or provider organizations in value-based care.
• Strong understanding of value-based care models and primary care operations.

Responsibilities

• Lead the implementation and deployment of value-based care models and practice transformation.
• Collaborate with providers to optimize clinical and administrative workflows using care pathways.
• Implement and optimize use of EHRs, patient portals, and care management tools.
• Develop population health programs and monitor quality performance against goals.
