Job summary

Aylo Health Services, Inc. is seeking a Network Performance Manager focused on Value Based Care to optimize contract performance across primary care networks.

The role guides practices through transformation, improves HEDIS measures, and aligns with population health strategies to drive sustainable, high-quality care. You will partner with practices, coach staff, analyze data for risk stratification and performance against quality and cost benchmarks, and support EHR and reporting workflows.

Qualifications

• Bachelor’s degree in Nursing, Public Health, Healthcare Administration, Business, or a related field is required.
• 4–7 years of experience in healthcare transformation, clinical quality improvement, and primary care operations.
• Experience working directly with primary care practices or in a practice facilitation role is highly preferred.
• Strong understanding of primary care delivery and value-based payment programs.
• Familiarity with HEDIS measures, MIPS, and CMS primary care initiatives such as MSSP and ACO REACH.
• Excellent coaching, communication, and relationship-building skills with clinical and non-clinical staff.
• Proficiency in interpreting and using data to drive quality improvement.

Responsibilities

• Partner with practices to assess baseline performance and co-develop transformation strategies.
• Guide practices through HEDIS quality measure improvement and payer-aligned initiatives.
• Coach clinical and administrative staff on quality improvement methodologies.
• Analyze data to inform decision-making, risk stratification, and progress toward benchmarks.
• Assist with EHR workflows and population health tools for care coordination.
• Facilitate virtual coaching sessions and site visits to share best practices.
• Act as liaison between practices and payers on performance and VBC initiatives.
• Support workflows, care team optimization, referral management, and related operations.
• Prepare leadership reporting packages related to contract performance.
