Complete-Health in Birmingham, AL is seeking a Patient Advocate to coordinate care for Value Based Care patients. The role includes telephonic and clerical support, appointment scheduling, and managing medical records in the Athena system.

You will assist with referrals, ensure timely appointments, and help with social services and Medicaid-related guidance while maintaining HIPAA compliance and delivering compassionate patient service.

Qualifications

• High school diploma or GED required; medical front office experience preferred.
• Strong communication, organizational, and documentation skills essential.
• Ability to work independently and as part of a team in a clinic setting.
• Proficient computer skills and familiarity with EMR systems and Athena.
• Knowledge of health insurance basics and HIPAA guidelines is preferred.

Responsabilités

• Serve as main contact for scheduling post-discharge patients and ER diversion.
• Upload hospital records to Athena and coordinate referrals and records with facilities.
• Assist with referrals, scheduling, and ensuring timely documentation in patient charts.
• Coordinate start of care with Home Health Agencies and SNFs when needed.
• Handle appointment reminders and ensure billing/documentation accuracy for VBC patients.
