Diverge Health is seeking a Practice Liaison to connect with partner practices and ensure ongoing access to Diverge Health’s technology and care solutions. You will collaborate with Network Development and Local Care teams to build relationships and maintain regular communication on patient care plans.

The role emphasizes onboarding, training, and education on value-based care, while translating insights into actionable steps and supporting practice performance improvements.

Qualifications

• Bachelor's degree in business, healthcare administration, or related field.
• 2–4 years of experience in a healthcare operational or customer-facing role (provider or payer).
• 2+ years' experience with a value-based care company preferred.
• Strong communication and presentation skills.
• Ability to travel within the community and support physician office visits.

Responsibilities

• Serve as the primary liaison between Diverge Health and partnered practices.
• Collaborate with Network Development and Local Care teams to establish and maintain relationships.
• Support onboarding, trainings, and access to Diverge Health technology solutions.
• Educate clinicians and staff on value-based care and Diverge Health incentives.
• Translate performance data into clear, actionable steps for improvement.
• Coordinate quarterly meetings to review incentive performance and quality scores.
• Navigate EMRs and Diverge’s Provider Portal; troubleshoot operational issues.
