Job summary

Kaiser Permanente in California seeks an experienced Registered Nurse for discharge planning and post-hospital care coordination. You will work with an MD and multidisciplinary team to assess patient needs, coordinate discharge plans, and ensure appropriate resource use while maintaining quality care.

The role requires strong collaboration, critical thinking, and adherence to regulatory standards, with a block schedule including every other weekend. California RN license and BLS are required.

Qualifications

• Two (2) years combined RN experience in an acute care setting or case management.
• Completion of an accredited RN training program that allows graduates to take RN license exam.
• Registered Nurse License (California) and Basic Life Support required.
• Demonstrated ability in utilization review/management, discharge planning or case management.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and multidisciplinary team to develop, implement and document plans of care.
• Recommends alternative levels of care and ensures compliance with regulatory requirements.
• Assesses high risk patients needing post-hospital care planning.
• Develops and coordinates discharge plans to meet identified needs.
• Communicates the plan to physicians, patient, family/caregivers, staff and community agencies.
• Reviews and coordinates hospital stay to ensure timely and efficient services.
• Participates in bed huddles and implements recommendations.
• Coordinates care planning rounds and patient/family conferences as needed.
• Participates in committees or other work projects as assigned.
