Qualifications

• California RN license and Basic Life Support.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and the multidisciplinary team to ensure continuity of care and appropriate resource use.
• Develops and coordinates discharge plans, and communicates them to patients, families, and providers.
• Coordinates transfer of patients to appropriate facilities and obtains necessary authorizations.
