Job summary

Kaiser Permanente is seeking an experienced Registered Nurse for a discharge planning/case management role in California. You will collaborate with MDs, families, and the care team to assess patient needs, coordinate appropriate post-hospital care, and ensure safe, cost-effective discharge plans.

Responsibilities include coordinating care plans, advocating for patients, and ensuring compliance with regulatory standards while maintaining high-quality patient outcomes.

Qualifications

• Two (2) years combined RN experience in an acute care setting or case management.
• Completion of an accredited RN program allowing graduates to take the RN license exam.
• Registered Nurse License (California).
• Basic Life Support Certification.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and the multidisciplinary team to develop individualized care plans.
• Recommends alternative levels of care and ensures compliance with regulations.
• Coordinates discharge planning and post-hospital care planning.
• Reviews and coordinates hospital stays to ensure timely delivery of services.
• Acts as liaison between inpatient facilities and referral facilities, providing case management.
