Job summary

Qualifications

• Two years RN experience in acute care or case management.
• Completion of accredited RN program.
• California RN license and current Basic Life Support (BLS).
• Strong communication and multitasking skills.

Responsibilities

• Collaborate with MD to coordinate admissions and screen for medical necessity.
• Recommend alternative levels of care when inpatient care is not required.
• Coordinate discharge planning with patient/family and care team.
• Monitor utilization and quality management activities.
• Coordinate transfers to outside facilities and obtain necessary approvals.
• Educate patients and families about discharge plans and resources.
