Job summary

Qualifications

• Two years RN experience in acute care or case management required.
• Completion of an accredited RN training program.
• California RN license and Basic Life Support required.

Responsibilities

• Coordinate and screen admissions with an MD to determine necessity for inpatient care.
• Recommend alternate levels of care when medical necessity is not met.
• Coordinate discharge plans with patients, families and care teams.
• Advocate for patients, ensure cost-effective, quality care, and multidisciplinarity.
• Transfer stable non-members to planned facilities; obtain required authorizations.
• Comply with AB 1203 and Post Stabilization notification; participate in utilization reviews.
• Participate in bed huddles and coordinate utilization management and discharge planning.
• Liaise between inpatient facilities and referral agencies to ensure continuity of care.
