Job summary

Kaiser Permanente in Downey, CA seeks an experienced Registered Nurse to coordinate and screen admissions and continued stays, and to recommend alternate levels of care when medical necessity for inpatient care is not met.

You will work with the MD and multidisciplinary team to develop discharge plans, educate patients and families, and ensure safe, cost-effective care across settings while complying with regulatory standards.

Qualifications

• Two years RN experience in an acute care setting or case management.
• Completion of an accredited RN training program that allows graduates to take RN license exam.
• CA RN license and Basic Life Support (BLS) required.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and patient/family to develop, implement and document appropriate plans of care.
• Recommends alternative levels of care and ensures compliance with federal, state and local requirements.
• Assesses high risk patients in need of post-hospital care planning.
• Develops and coordinates discharge planning to meet identified needs.
• Communicates the plan to physicians, patient, family/caregivers, staff and community agencies.
• Reviews and coordinates the hospital stay to ensure services are delivered timely.
• Coordinates care planning rounds and family conferences as needed.
• Participates in committees or projects as assigned.
