Job summary

Kaiser Permanente seeks an experienced Registered Nurse for case management to coordinate admissions, screen appropriateness for inpatient care, and guide safe discharge planning. You will work with physicians, families and a multidisciplinary team to ensure cost-effective, quality care and timely services.

The role emphasizes regulatory compliance, documentation, and collaboration across departments to optimize patient outcomes and resource use.

Qualifications

• Two (2) years combined RN experience in an acute care setting or case management required.
• Completion of an accredited RN training program that allows graduates to take RN license exam.
• Working knowledge of Utilization review/management, discharge planning or case management.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and the multidisciplinary health care team to ensure continuity and quality of care.
• Recommends alternative levels of care and ensures compliance with requirements.
• Develops and coordinates discharge plans to meet patient needs.
• Communicates the plan to physicians, patients, families and community agencies.
• Coordinates transfer of patients to appropriate facilities and maintains required documentation.
• Participates in utilization management, transfer coordination, discharge planning and obtaining authorizations.
