Job summary

American Family Care seeks an Advanced Practice Provider (PA/NP) to join a franchised AFC location in Santa Clara, CA. The APP will perform comprehensive assessments, order labs and imaging, and coordinate care with physicians and staff to maintain smooth patient flow and high satisfaction.

The role requires California PA/NP licensure, board certification (ANCC/AANP/NCCPA), and the ability to multitask in a busy urgent care setting with flexible scheduling.

Qualifications

• ANCC, AANP, or NCCPA board certification required (ER/UC exp preferred).
• Active PA or NP license in California, plus DEA and state-controlled substance certificates.
• Ability to multitask and work independently.
• Excellent communication with staff, patients, and families.
• Experience in urgent care or ER settings is preferred.

Responsibilities

• Perform complete, detailed health histories, review records, develop assessments, and order labs/imaging as appropriate.
• Formulate medical/nursing diagnoses and refer to facilities or resources.
• Institute emergency measures and stabilize patients in critical situations.
• Interpret and analyze patient data to determine status and treatment.
• Provide guidance on health care and health promotion to patients and families.
• Other duties as assigned.
