American Family Care Chino is seeking an Advanced Practice Provider (APP) to deliver high-quality urgent care in a franchised AFC location. You will work with other providers to ensure efficient patient flow and high patient satisfaction while operating within your scope and approved agreement.

Duties include comprehensive histories, medical assessments, diagnostic planning, and appropriate referrals. The role emphasizes emergency readiness, patient education, and independent, multi-disciplinary

Qualifications

• Excellent communicator with staff, patients, and family.
• 1 year Urgent Care experience required.
• Professional appearance and attitude.
• ANCC, AANP, or NCCPA Board Certified.
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required.
• Able to multi-task and work independently.

Responsabilités

• Perform complete health histories, review records, develop medical assessments, and order labs/imaging as appropriate.
• Formulate medical diagnoses and referrals to facilities and community resources.
• Institute emergency measures and stabilization in emergencies.
• Interpret data to determine patient status and management.
• Provide health care instructions and guidance to patients and families.
• Other duties as assigned.
