InnovAge is hiring a Registered Nurse to deliver skilled nursing care in clinic and in-home settings, partnering with an interdisciplinary team to develop and execute individualized plans of care for frail and elderly participants.

This role requires a valid RN license, First Aid and BLS, and at least one year of geriatric experience. Travel to participant homes is expected as part of care delivery.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Current First Aid and BLS certifications (required prior to hire).
• 1+ year experience with frail or elderly populations.
• For in-home assignments: valid driver's license, auto insurance, and acceptable driving record.

Responsabilités

• Perform comprehensive assessments and reassessments, update Plans of Care.
• Administer medications, treatments, wound care, and other skilled nursing interventions.
• Triage calls to determine priority and route/escalate as needed.
• Provide medication administration and training for self-administration; promote adherence.
• Educate participants and families on conditions, medications, equipment, and safety.
• Coordinate outside visits, diagnostics, and hospital admissions; arrange transportation when needed.
• Document care thoroughly in the medical record; support quality management activities.
