InnovAge is seeking a Registered Nurse to provide skilled nursing care in both clinic and in-home settings. The RN will collaborate with the interdisciplinary team to develop and execute individualized Plans of Care, ensuring safe, effective care for frail and elderly participants.

Responsibilities include comprehensive assessments, medication administration, education for participants and families, and care coordination across settings.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Current First Aid and BLS certifications (AHA/Red Cross) required prior to hire.
• 1+ year experience working with frail or elderly populations.
• For in-home care: valid driver's license, transportation, and auto insurance as required by law.

Responsabilités

• Perform comprehensive assessments, reassessments, and monitoring to adjust care plans.
• Implement provider orders, med administration, wound care, and skilled nursing interventions.
• Triage calls to determine priority and route/escalate as needed.
• Educate participants and caregivers on conditions, medications, equipment, and safety.
• Coordinate outside visits, diagnostics, and hospital admissions; arrange transportation.
