Qualifications

• Specialized Diploma graduate of an accredited school of nursing.
• 3 years as a Registered Nurse in an acute care setting.
• Effective communication and partnership with patients and families.
• Patient-centered practice with cultural sensitivity.
• Care coordination across multidisciplinary teams.
• Knowledge of discharge planning and patient safety.
• Active RN license in state of practice.

Responsibilities

• Acts as a resource, role model, and clinical leader for the patient care team.
• Evaluates patient needs and makes care recommendations.
• Monitors staff schedules to ensure adequate staffing to meet patient care needs.
• Collaborates with the medical staff to facilitate consistency of interdisciplinary plans of care.
• Assesses patient needs and makes care recommendations based on physical, psychosocial and cultural needs.
• Facilitates timely patient throughput and discharge planning.
• Provides patient care support when necessary.
