Job summary

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• 1–2 years nursing experience; Home Health Hospice or triage experience preferred.

Responsibilities

• Triage all patient and caregiver calls; assist in solutions, palliation of symptoms, and education.
• Serve as liaison with physician offices on behalf of the care team.
• Provide immediate emotional support to families at the time of death call.
• Coordinate changes in patient level of care; communicate with insurance case managers.
• Assist with pharmacy issues and document discussions in EMR; coordinate equipment as needed.
