Responsibilities

• Provides quality nursing care to life-limiting illness patients and supports families.
• Participates in interdisciplinary team to develop and implement individualized plans of care.
• Evaluates patient/family response and updates care plans; educates on infection control protocols.
• Documents accurately in medical records and supports quality improvement programs.
• Supervises Patient Care Partners, Unit Receptionists and volunteers as needed.
• Attends to patient and family at the time of patient’s death.
