Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to provide compassionate nursing and supportive care to terminally ill patients and families in their homes or facility settings in Maryland. The role requires scheduling follow-ups, collaborating with an interdisciplinary team, and delivering patient-centered education to families and staff.

The ideal candidate holds a Diploma/ADN/BSN and Maryland RN licensure, with at least two years of nursing experience.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Maryland State RN license is required.
• Two years of nursing experience, or an equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs, including physical, emotional, spiritual and mental status.
• Prepares and implements the care plan for the patient/family unit in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain the highest level of care in collaboration with the primary physician.
• Plans and supervises the delivery of care by the HHA.
• Updates the primary physician and interdisciplinary team regularly; communicates at care conferences.
• Uses problem-oriented charting to ensure continuity of care in home or facility settings.
• Educates family members, facility staff, hired caregivers on patient care skills including pain control, injections, IVs, dressing changes, hygiene, and vital signs as ordered.
• Ensures arrangements for equipment necessary in the facility setting are made available.
• Attends to patient, family and facility at the time of death.
