Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to identify home care needs for patients and families, developing and implementing care plans with the Interdisciplinary Group.

The role requires Maryland RN licensure and a BSN/ADN, with 2 years of nursing experience. Strong infection control, time management, and family-centered care are essential. You will educate families, supervise home health aides, and coordinate with physicians.

Qualifications

• Current Maryland State license as a Registered Nurse.
• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledgeable of universal precautions for infection control and practices safe nursing standards.
• Ability to be patient and family focused.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain highest level of care.
• Plans and supervises the delivery of care by the HHA.
• Updates primary physician and interdisciplinary team regularly.
• Uses charting to ensure continuity of care in home/nursing settings.
• Teaches family members and caregivers patient care skills (medication, injections, IVs, wounds, vitals).
• Arranges for equipment and attends care plan meetings.
