Job summary

Elara Caring is seeking a Speech-Language Pathologist to deliver high-quality patient care in home settings across New Hampshire. You’ll evaluate patients, develop and implement plans of care, and collaborate with physicians and care teams to ensure continuity of services.

The role may require occasional on-call hours and travel within the branch coverage area. A Master’s degree in Language Pathology and a current SLP license are required.

Qualifications

• Master’s degree in Language Pathology.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.
• Able and willing to travel 50%.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Promotes Elara Caring’s philosophy, mission statement and policies to ensure quality of care.
• Evaluates the patient and develops a plan of care.
• Admits the patient and assesses therapy needs according to regulatory guidelines; conducts initial evaluation and re-evaluation.
• Initiates patient safety plans with family and community resources.
• Communicates significant findings to supervisor and care team.
• Implements and updates the Plan of Care and discharge planning.
• Writes physician orders for additional visits as needed.
• Incorporates patient goals into therapy plans and updates as issues arise.
• Performs direct Speech-Language pathology services per standards and physician orders.
• Selects materials appropriate to patient needs and trains patients/caregivers at home.
• Maintains accurate therapy notes and logs.
• Handles physically demanding environment and potential irregular hours including on-call.
