Job summary

Elara Caring seeks a Speech-Language Pathologist to deliver home-based care, evaluating patients, developing plans, and coordinating therapy across the care team. The role emphasizes collaboration, safety planning, and comprehensive documentation in a home-health setting.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the work state.
• Minimum one year of clinical SLP experience.
• Willingness to travel within branch/office coverage area (up to 50%).
• Ability to lift up to 50 pounds and transfer patients.

Responsibilities

• Promotes Elara Caring’s mission and policies to ensure quality of care.
• Evaluates patients and develops a plan of care.
• Admits patients and conducts initial therapy evaluations; re-evaluates as needed.
• Initiates safety plans and coordinates with family and community resources.
• Communicates findings to supervisor and care team.
• Implements and updates the Plan of Care; writes orders per policy.
• Incorporates patient goals into therapy and documents progress.
• Provides direct Speech-Language pathology services per standards.
• Selects materials and educates patients/caregivers for home therapy.
• Maintains accurate therapy notes and logs; coordinates with physicians.
