Job summary

Elara Caring is seeking a Speech-Language Pathologist to provide in-home therapy across our branch network. The role focuses on evaluating patients, developing tailored Plans of Care, and delivering high-quality speech-language pathology services in patients’ homes.

Travel within the coverage area is required, with some days spent in the field and others coordinating care with the branch team. Applicants must hold a Master’s degree in Language Pathology, an active SLP license, and at least one

Qualifications

• Master’s degree from an accredited program in Language Pathology.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.

Responsibilities

• Promotes Elara Caring’s philosophy, mission statement and policies to ensure quality of care.
• Evaluates patient needs and develops a plan of care.
• Admits patient and assesses therapy needs per regulatory guidelines; conducts initial and follow-up evaluations.
• Initiates patient safety plans and coordinates with family and community resources.
• Communicates findings to supervisor and care team.
• Implements and documents Plan of Care and discharge planning.
• Writes physician orders for additional visits as needed.
• Delivers direct Speech-Language pathology services.
• Selects appropriate materials and educates patients/caregivers at home.
• Ensures notes and logs are accurate and submitted per policy.
