Job summary

Elara Caring is seeking a Speech-Language Pathologist to support home-based patients. You will evaluate, treat, and coordinate speech-language therapy in accordance with physician orders and established guidelines.

Travel up to 50% is required, with lifting and transfer duties as part of daily patient care. The role requires a Master's in Language Pathology, active SLP license, and at least 1 year of clinical experience.

Qualifications

• Master’s degree from an accredited program in Language Pathology.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance.
• Must be able and willing to travel 50%.

Responsibilities

• Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
• Evaluates the patient and develops a plan of care.
• Initial therapy evaluation, re-evaluations and follow-up assessments per guidelines.
• Initiates plan for patient safety and coordinates with family and community resources.
• Communicates findings to supervisor, physician and team members.
• Implements and develops the Plan of Care and ensures proper discharge planning.
• Writes physician orders to cover visits and plan changes.
• Incorporates care goals into therapy notes and updates the Plan of Care as needed.
• Performs direct Speech-Language pathology services per standards of practice.
• Selects diagnostic and therapeutic materials and educates patient/caregiver.
• Ensures notes and logs are accurate and submitted.
• Occasional high-stress environment and physically demanding tasks.
