Job summary

Elara Caring is seeking a Speech-Language Pathologist to deliver in-home therapy across the Kennebunk area. You will assess patients, develop plans of care, and work with families to support communication goals.

This role requires a Master’s in Language Pathology, a current SLP license, and at least one year of clinical experience. Travel up to 50% is required, with lifting up to 50 pounds, and you will join a team dedicated to compassionate, home-based care.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the practicing state.
• Minimum one year of experience as an SLP in a clinical setting.
• Ability and willingness to travel within branch/office coverage area (50% travel).

Responsibilities

• Evaluate patient to determine rehabilitation needs and develop a plan of care.
• Admit and assess speech-language therapy needs according to guidelines (OASIS and PPS).
• Initiate plan for patient safety using patient, family, and community resources.
• Communicate significant findings and changes to supervisor and team.
• Participate in the implementation and development of the Plan of Care and discharge planning.
• Write physician orders for visits and changes to the Plan of Care.
• Incorporate patient care goals into therapy notes and revise as needed.
• Perform direct Speech-Language pathology services per standards and physician approval.
• Select materials and instruct patients and/or caregivers in home therapy program.
• Ensure notes/logs are accurate and submitted per policy.
• Work in a physically demanding, high-stress environment; travel up to 50%.
