Job summary

Elara Caring is seeking a Speech-Language Pathologist to join its home-based care team. You will evaluate and treat patients in their homes, develop plans of care, and coordinate with physicians and families to ensure quality outcomes.

The role requires active state licensure, a Master’s in Language Pathology, at least 1 year of clinical SLP experience, and the ability to travel up to 50% with a personal vehicle. Pay ranges from $65 to $80 per hour, depending on experience and location.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the work state.
• Minimum one year of clinical SLP experience.
• Able and willing to travel within branch coverage area.
• Dependable vehicle with valid driver’s license and auto insurance.
• Willing to travel up to 50%.
• Ability to sit, stand, bend, lift and move intermittently; lift 50–100 lbs.

Responsibilities

• Promotes Elara Caring’s mission and care policies.
• Evaluates patients and develops a care plan per findings.
• Conducts initial evaluations and re-evaluations per guidelines.
• Initiates patient safety plans with family and community resources.
• Communicates changes to supervisors, physicians, and team.
• Coordinates Plan of Care and ensures proper discharge planning.
• Writes orders to cover visits and changes to the Plan.
• Integrates goals into therapy notes and updates Plan of Care.
• Performs direct Speech-Language pathology services per standards.
• Selects materials and instructs patients/caregivers for home therapy.
• Ensures notes and logs are accurate and submitted per policy.
• Adapts to physically demanding, high-stress environment.
• Manages full range of motion tasks including lifting.
• May require irregular hours including on-call.
