Job summary

Elara Caring is seeking a Speech-Language Pathologist to provide home-based therapy and evaluation services. The role involves travel within the branch/office coverage area, with a focus on delivering high-quality care in patients’ homes.

Requires a Master’s in Speech-Language Pathology and an unrestricted SLP license, plus at least 1 year of clinical experience. Travel 50% and a dependable vehicle are required.

Qualifications

• Master’s degree in Speech-Language Pathology is required.
• Unrestricted SLP license in the state of work is required.
• At least 1 year of clinical SLP experience is required.
• Must be willing to travel 50% within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and auto insurance.

Responsibilities

• Evaluates patient to determine rehabilitation needs and develops a plan of care.
• Admits patient and assesses speech-language therapy needs per regulatory guidelines.
• Performs direct Speech-Language pathology services and documents therapy notes.
• Communicates findings and changes to supervisor and care team.
• Plays a role in discharge planning and coordination with physician orders.
