Job summary

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager for the St. Cloud area.

The RNCM will lead the patient’s hospice care team, develop and manage the Plan of Care with the Hospice Medical Director, patient and family, and facility staff, delivering day-to-day patient care and coordinating treatments. Responsibilities include administering medications, maintaining records, and providing emotional support to patients and families, with on-call duties and interdisciplinary

Qualifications

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Multistate license allowed where applicable.
• Ability to pass DHS background, consumer report, and drug test.

Responsibilities

• Provides day-to-day patient care and leads the hospice care team.
• Develops and revises the Plan of Care in collaboration with the team.
• Administers medications and treatments as prescribed.
• Maintains patient records and coordinates with physicians and care partners.
• Provides emotional support and counseling to patients and families.
• Participates in on-call visits and interdisciplinary group meetings.
