Job summary

The Registered Nurse Case Manager (RNCM) at St. Croix Hospice leads each patient’s hospice care team and manages their Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff.

The RN Case Manager provides day-to-day patient care, administers medications, maintains patient records and directs other members of the patient care team to meet changing needs with a focus on comfort and quality of life.

Qualifications

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Nurses with a multistate license can practice in other states without extra licenses while maintaining their RN license in primary state.
• Graduate of a professional school of nursing and licensed in state(s) of practice.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License and access to an automobile in usable condition.

Responsibilities

• Leads each patient’s hospice care team and manages their Plan of Care with the Hospice Medical Director, patient, family and staff.
• Provides day-to-day patient care, administers medications, and maintains patient records.
• Coordinates care with physicians, interdisciplinary teams and community partners.
• Ensures HIPAA compliance and patient confidentiality.
• Offers emotional support to patients and families and arranges necessary equipment.
