Job summary

The Registered Nurse Case Manager (RNCM) at St. Croix Hospice leads each patient’s hospice care team, developing the Plan of Care in collaboration with the Hospice Medical Director, patient, and family.

You will provide day-to-day nursing care, coordinate with physicians and the interdisciplinary team, educate families, ensure HIPAA compliance, and participate in on-call visits as needed.

This role emphasizes comfort, quality of life, and compassionate support in home and facility settings.

Qualifications

• Current RN license in state(s) of practice.
• Ability to pass DHS background, drug test, and driving record checks.
• Hospice or home-health experience is preferred.

Responsibilities

• Lead the patient’s hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, and family.
• Provide day-to-day nursing care and administer medications as prescribed.
• Coordinate care with physicians and interdisciplinary team; educate patients and families; maintain accurate documentation; ensure HIPAA compliance.
• Attend on-call visits and participate in quality assessment and performance improvement activities.
