Job summary

St. Croix Hospice in Saint Cloud is seeking an experienced RN Case Manager to lead each patient’s hospice care team and develop the Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff.

You will provide day-to-day patient care, manage treatments, administer medications, maintain records, coordinate with physicians and interdisciplinary teams, and support patients and families to improve comfort and quality of life.

Qualifications

• Requires active RN license in state(s) of practice.
• Graduate of professional nursing program; licensed/certified in practice states.
• Ability to pass background check and drug screen.

Responsibilities

• Lead the patient’s hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, family and facility staff.
• Provide day-to-day patient care and manage treatments.
• Administer medications and maintain patient records.
• Coordinate care with physicians and interdisciplinary teams.
• Support patients and families to ensure comfort and quality of life.
