Signify Health, part of CVS Health, is seeking a Full Time In-Home Traveler Physician to conduct in-home health evaluations across a defined coverage area. The role requires extensive travel, up to 100%, with overnight stays as needed.

You will perform comprehensive assessments using an iPad, review medical history, and provide diagnostic services without altering current regimens. You will join a national network of clinicians dedicated to improving access to care and health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses as required by business needs.

Responsabilités

• Provide In-Home Health Evaluations for health plans, engaging with members in their homes.
• Travel up to 100% across defined coverage area as required.
• Perform 45‑minute visits including health questions, med history review and physical evaluation.
• Conduct virtual visits as needed.
• Deliver Signify Health services such as Diagnostic & Preventive Services per program guidance.
