Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations across a defined coverage area. The clinician travels up to 100% of the time, conducts 45-minute visits, reviews medical history, performs physicals, and may arrange optional tests per health plan guidance.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Conduct in-home health evaluations (45 minutes) with health history review and physical evaluation.
• Coordinate with health plan to determine necessary diagnostic tests (diabetes eye exams, spirometry, etc.).
• Utilize an iPad and clinical workflow; no prescribing or lab orders required.
• Travel locally and within licensed states as needed.
• Provide diagnostic and preventive services as indicated.
• Engage with patients to connect them with appropriate care.
