Job summary

St. Croix Hospice is seeking a dedicated Hospice Aide (CNA) to join our in-home care team in Frederic, WI. You will provide compassionate personal care under a Registered Nurse, enhancing comfort and dignity for patients at the end of life.

This full-time role requires 100% travel to patient homes, a current CNA certification, a valid driver’s license, and dependable transportation. Excellent teamwork and accurate documentation are essential to success.

Qualifications

• Current CNA certification in the state of practice.
• Valid driver’s license and reliable transportation.
• Provide personal care including bathing, skin and nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Promote patient comfort, dignity, and safety.
• Travel to patient homes throughout the day and provide care as scheduled.
• Maintain accurate documentation of services provided.

Responsibilities

• Deliver hands-on, one-to-one care in patients' homes.
• Support daily living activities to improve comfort and quality of life.
• Ensure safety and dignity during care encounters.
• Communicate effectively with the care team to coordinate services.
