Job summary

St. Croix Hospice is looking for a dedicated Hospice Aide (CNA) to join our interdisciplinary team, delivering compassionate, in-home care to patients and families in Wisconsin.

Under RN supervision, you’ll assist with personal care and daily activities to support comfort and dignity at the end of life. The role requires a current CNA certification in the state, a valid driver's license, and reliable transportation.

Qualifications

• Current CNA certification in the state of practice.
• Valid driver’s license and clean driving record.

Responsibilities

• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Promote patient comfort, dignity, and safety.
• Travel to patient homes and provide care as scheduled.
• Maintain accurate documentation of services provided.
